Parents/Caregivers Please Note: CIS is a school-based program. In order to participate in CIS, a student must attend one of the schools listed below. In addition to attending one of these schools, there are eligibility requirements that students must meet in order to receive services. If you are a parent/caregiver interested in learning more about CIS and the eligibility criteria, please contact your child’s school directly and ask to speak with the Communities In Schools (CIS) Site Coordinator ; or you may complete this Parent Inquiry Form and a representative from CIS will contact you.
